Dementia Care & Understanding: A Family Pathway – Phase 4
Phase 4 focuses on life after a dementia diagnosis and the practical, emotional, and environmental adjustments that follow. This phase supports families to move beyond the diagnosis itself and begin building sustainable routines, wellbeing strategies, and home adaptations that support everyday life.
A core element of Phase 4 is processing the diagnosis. Families learn how a diagnosis can affect emotional wellbeing, identity, relationships, and family dynamics, and why reactions such as shock, denial, relief, fear, and grief are common and often cyclical. The phase emphasises open communication, inclusion of the person with dementia, and acknowledging emotional impact without avoidance.
Phase 4 also provides family-level understanding of dementia types, explaining why the type of dementia matters and how it shapes symptoms, risks, progression, and care planning. The focus is on clarity rather than labels, helping families make sense of behaviours and prepare realistically for what may come next.
Practical application is central to this phase. Families are supported to establish daily routines, make early lifestyle adjustments, and introduce home adaptations that improve safety, comfort, and independence. The phase covers equipment access, occupational therapy involvement, and planning for future needs before crisis points arise.
Phase 4 also introduces early legal and financial actions, helping families understand why addressing matters such as LPAs, benefits, financial protections, and future planning at this stage reduces stress and conflict later.
This phase equips families with the knowledge and tools needed to stabilise daily life, protect wellbeing, and build a clear progression plan following diagnosis.
